This largely depends on your needs and your personal level of comfort.

Daily food
In Paris you can eat for relatively little if you're willing to shop around and know how to cook.

At my university, and others I know, you can get a pretty decent meal (people complain, but I find it pretty good for cafeteria food) with a starter, main course and dessert for about 3€10.

Takeaways generally cost at least 5€00, occasionally some tiny hole-in-the wall ethnic restaurants do a dish of the day for 5-6€00, but you have to know where they are, regular restaurant meals are usually from at least 9€00 and up for one dish.

Transportation
Unless you live close to your school, you will probably need a transport card. We usually get a year card and pay it each month.

Renting
This is the most expensive in Paris. The suburbs will be cheaper, but you will be paying more in transport.
For a room or studio you may be paying at least 5-600€ a month. If you are fortuneate and you apply early, you may get a university room. this may be cheaper, but they are hard to get. You may also be able to apply for a housing allowance to help pay the rent, but this is decided on a case by case basis.

Electricity, phone and internet
Electricity bills will depend on what heating you have, as electric heating can be very expensive, and necessary if you are renting an old appartment - it is something to check when you choose your accommodation.
The rest depends on what you need and what you are willing to pay, but if you really shop around, you could find some good deals.

4. Do i require health insurance if yes what is the avrage cost and the cost of doctor visits ect.

If you are a student under 28, then this is obligatory and you pay for this when you pay your university fees unless you can prove you already have medical coverage.
To see a generalist is probably about 23€, a specialist will cost more.
