For the application we need the following:

2 readable application forms properly filled out in German including the correct telephone number and address.
2 newly taken biometric passport photos.
The applicant鈥檚 original identity card (CIN) and 2 photo-copies.
Original passport with 2 copies of the 1st and third pages.
The passport must be valid for at least 6 more months and have 2 blank pages.
A properly self-addressed envelope including the required postage.
