I would recommend you google for insurance providers in Bahrain and contact them to get the required info, a few of the well know are BUPA, AXA, Cigna.

And it will cost a lot for older people if you want good coverage.

No. That depends on a lot of factors. But a top quality insurance, which covers everything, for people of that age.... Don't expect less than 1000 BD per person...... Maybe even 1500-2000 BD.
